There is one more registration
session scheduled for the 2009 season. All associations except
Salem may also
fill out the forms below and mail them in with full payment to
register. Mail in option is no longer open to Salem because we
are trying to manage caps that have been placed in Salem in the
spirit of a controlled pace for growth and cost.
Save time in line at registration by filling out your forms in
advance. It is best to have your child with you at registration so
they can be weighed.

birth certificate. A hospital issued birth certificate (the kind
with the footprint stamps) will not

Registration:

MVPW is dedicated to match teams up by schools. In the event a split or
fill needs to occur, there is a possibility that you will not play
with your friends. There are no refunds for this type of event. |

Team Registration is
closed after the first 25 players register. All players after the first
25 will be placed on a waiting list or non-school specific team. MVPW will create a
second team if there is enough
participants. Teams are created by Elementary school -> Middle
School -> High School. Lower team rosters allow all players the opportunity to develop and have
fun. |

If you choose to use a
personally purchased helmet, MVPW will require a waiver of
liability. |

Refund policy
Although MidValley Pop
Warner has a policy of NO REFUNDS, from time to time, the Board
of Directors does consider refunds under the following
guidelines:
•All refund requests are subject to approval, by vote of a
quorum of the Board of Directors.
•All refund requests must be submitted in writing to the Board
of Directors. No refunds are automatic or compulsory.
•Any participant who drops from the program for a valid medical
reason may be allotted a prorated refund of their registration
fees. Proof of such medical condition
may be required.
•Any refund request must be submitted before July 31st. There
shall be no refunds permitted after this date, except for
medical reasons, as stated above.
•Any refund for reasons other than those stated above shall be
at the sole discretion of the Board of Directors.
MVPW Participation Scholarship Policy
Fifty percent participation
scholarships will be awarded based on a standard income / family
size matrix. Proof of eligibility is required: Participation scholarships will not be awarded
without proof of eligibility prior to or at time of registration. MVPW recognizes the need for
privacy
and will treat all Participation scholarship
requests with extreme
discretion. No player will be left behind due to inability to pay the
league fees. We'll find a way
so they can play. No player or cheerleader will be turned away because
of family financial restrictions.
Parent Volunteer Policy
MVPW is an all-volunteer organization. All families of players and
spirit leaders are required to volunteer their time a minimum of 10 hours
during the year. We recognize many parents will volunteer more and
with out those parents there is no MidValley Pop Warner and we all
thank you. Active committee work fulfills your 10 hr volunteer
commitment. Ten-hour commitment does not have to be
completed during the season but can be done sometime during the
year.
MVPW will again offer a volunteer "Opt-Out" option to
parents that will not be able to participate in the tasks involved with
making the league and season a success. The volunteer opt-out fee
is $210.
Proprietary Rights:
(from the PWLS Policies and Procedures Manual)
Section
1
The Corporation retains sole and exclusive rights to use of the
name “Pop Warner” in simple or compound appearance (e.g., “Pop
Warner Football”) as well as all logos, emblems and other
designs and styles of the Corporation, including any copyright,
trademark, service mark or similar matter. Such rights shall
include use in all forms of media and are hereinafter
collectively referred to as “Proprietary Rights”. The
Corporation shall vigorously protect against any infringement
or other misappropriation or misuse of its Proprietary Rights.
Section
2
The Field Membership, Associations, RD's, VRD's or RCDC's shall
not have the power to enter into contracts on behalf of the
Corporation or bestow titles of any type upon the Field
Membership and shall not infringe upon, through use or
otherwise, the Proprietary Rights of the Corporation.
Section 3
Upon the request of the Executive Director, all employees,
volunteers, officers, directors, and members of committees
shall, in writing, acknowledge their understanding and agreement
that all information they obtain in the course of employment or
service is the property of the Corporation and that it shall not
be made available to any person or business without the consent
of the Executive Director.
